Simple Appointment Calendar (scheduler) Script?

I am looking to incorporate a very simple scheduler (appointment calendar) on a web site. I have found some scripts (PHP Ressource Index) that do this but they are usually too complex for what I need. I want a PHP script that uses SQL.

Basically I want a script where people select a day, then select times (for example 3:00 - 3:30) and put a name, and maybe an email. It is for a massage therapist that has many clients and she wants them to sign up online.

what is the admin like? the online demo only shows the calender and not how to add elements.
