Im trying to write a sipmle questionnaire in php. unfortunately im not experienced in doing staff like this. can anyone recomend any site where i could find any samples of codes used??
i would like to see how people approached it.

They are not discussed often, but IIRC quite a few methods have been discussed.
Try using the keyword “survey”.
There’s a whole slew of tools and services out there, both free and paid.
