I don't, but I would suggest other options, like getting a CampaignMonitor account and using that service for the mailouts. Let them absorb the responsibility for mass mailouts, as that can make your own ISP twitchy. You can style the CM form any way you like. And you can set up email templates in CM that the client can use easily to enter content and send mail. You also get really comprehensive stats on the emails after they've been sent, too.
