Hello, is their a way to make this pdf editable on a webpage
like this one http://smiledentalpractice.com/pdf/smilledentalpractice_filloutform.pdf
and not be an actual form. Would it be something that I need edit in Adobe Acrobat?
PDFs can be made to have editable regions in Acrobat, but I’ve always found it a bit clunky, and the end user may mot be able to save the changes, so often has to print them out to be of any use.

Is there a special reason why you want it to be a PDF and not a html form?
