Online PHP Contact Form Builder

Hello,

Can anyone recommend an online email contact form builder? I am looking for a form generator that would let me create a custom form with at least the following features:

-client side validation
-server side validation (I want it in php)
-some kind of CAPTCHA or anti spam
-provides all the source files/code. I want everything hosted/processed on my site. No third party processing the form, etc.

I’m willing to pay a fair price for the form. I’m not expecting a free service.
