CSS Help: Background image not loading on webpage

I cannot figure out why the background image is not loading on the webpage here:

In fact, on the above webpage the styling has become all messed up. I had created a similar form earlier which looked something like this http://audiquattromumbai.tigersportsmarketing.com/attending/index.html (and thats how I want it on the first link)

So here I want to understand why the background image is not loading + why is the formatting messed up
