Affiliate script for tracking inquiries?

Is anyone aware of an affiliate script that can track referrals as opposed to sales?

For example, for a product that is sold off-line from leads gathered online, is it possible to have the affiliate link direct the user to an inquiry form and track who referred them so they can get commission if the lead results in a sale?

Ideally the affiliate would be able to see the number of leads in their affiliate stats and could be manually updated if a sale occurs?

Not really, the best I could come up with is to use an off-the-shelf package (I’m using JAM), and am getting a developer to parse the affiliate ID from the URL and add as a hidden field to the form so we can manually track it.
