Simple way to collect email addresses and email PDF files

I’m tearing the web up looking for this simple thing. All I want to be able to do is receive a form from a site visitor and after they submit it, they are emailed a PDF file. I found this:

which looks like it will work… Is there anything else?

I plan to use a site builder like Weebly or Yola, and they allow custom html, so I thought I could use a site like jotform.com to generate the form perhaps, and run form processor pro from one of my godaddy domains…

It’s easy enough to create a form-to-email script that sends an email to the visitor. That email could contain a link for downloading the PDF file. Not what you are asking for, but that would be very easy.
