how do you manage browser caching ??

how much do you keep caching period for js | css | images ??

I am more concern about
"how to show user updated content everytime and also do browser caching ??

After setting browser caching how do you show updated content ??

one more thing i would like to ask is
Do you prefer to do caching through htaccess or through meta tags ?? which is better ??

Does appending querystring in the url helps in loading new file everytime and also do caching

domain.com/script.js?ver=001
