I’m looking to purchase a PHP/mySQL ad management script. It doesn’t need to be extremely complex, just reliable. I guess if there was a free option out there that had great reviews and a long history, that would be fine too. I generally assume a script that costs $100 or $200 from an established company would be more reliable.
Does anyone have any recommendations?

Do a search for banner rotator if that is all you need, there should be tons of decent ones out there.
Can’t recommend any specific ones myself unfortunately.
openx looks very interesting but it appears to be a bit overkill. We just have simple 468x60 ad sizes for some of our community members who wish to advertise their products. We need a simple rotation program to install and that rotates ads and tracks views/clicks.

You can try OpenX , Kamads and PHPads as these three are Open Source 
I use OpenX, it works very well for large sites but can be a little much for a smaller site.
