I’d use “which of the following subjects”, as “below” is visual.
No one has added this option for including “most”:
“Which of these subjects most interest you? (Choose up to 5)”

After reading the myriad of responses, I will weigh-in.
Always, I prefer to be concise. So I propose a declarative form:
Choose up to 5 that interest you.
If you really wanted to know how interesting the person thought each subject, I would look at having possibly two sections - one identifying which subjects are interesting (the pick 5) and then - rate these 5 topics in order of preference.
But I think I would understand Which subjects interest you? (Choose up to five.)
as picking the ones I am most interested in.
