Free UML Tools? (Architecture to Half-Stack-Implementation)

Hello,

Does anyone know of a free UML tool that allows you to graphically design a database, and then create/modify that database?

Even better, does anyone know of a tool that does the above plus implements a Object Oriented series of classes to interact with the database in a server side language like PHP?
