Contact Form not working

Hi there,

I’m having issues with the Contact section of my website - after filling out the form and clicking “submit”… I don’t receive an e-mail. Could really use some help with this.

My site is StephanieLange.com

This is the code for the section:

<!-- To configure the contact form email address, go to mail/contact_me.php and update the email address in the PHP file on line 19. -->
                    <!-- The form should work on most web servers, but if the form is not working you may need to configure your web server differently. -->
                    <form name="sentMessage" id="contactForm" novalidate>
                        <div class="row control-group">
                            <div class="form-group col-xs-12 floating-label-form-group controls">
                                <label>Name</label>
                                <input type="text" class="form-control" placeholder="Name" id="name" required data-validation-required-message="Please enter your name.">
                                <p class="help-block text-danger"></p>
                            </div>
                        </div>
                        <div class="row control-group">
                            <div class="form-group col-xs-12 floating-label-form-group controls">
                                <label>Email Address</label>
                                <input type="email" class="form-control" placeholder="Email Address" id="email" required data-validation-required-message="Please enter your email address.">
                                <p class="help-block text-danger"></p>
                            </div>
                        </div>
                        <div class="row control-group">
                            <div class="form-group col-xs-12 floating-label-form-group controls">
                                <label>Message</label>
                                <textarea rows="5" class="form-control" placeholder="Message" id="message" required data-validation-required-message="Please enter a message."></textarea>
                                <p class="help-block text-danger"></p>
                            </div>
                        </div>
                        <br>
                        <div id="success"></div>
                        <div class="row">
                            <div class="form-group col-xs-12">
                                <button type="submit" class="btn btn-success btn-lg">Send</button>
                            </div>
                        </div>
                    </form>

And this is the contact_me.php code, *note, I did change it to the e-mail address I’d like to receive mail to but didn’t want to include my e-mail address in post:

<?php
// Check for empty fields
if(empty($_POST['name'])  		||
   empty($_POST['email']) 		||
   empty($_POST['phone']) 		||
   empty($_POST['message'])	||
   !filter_var($_POST['email'],FILTER_VALIDATE_EMAIL))
   {
	echo "No arguments Provided!";
	return false;
   }

$name = $_POST['name'];
$email_address = $_POST['email'];
$phone = $_POST['phone'];
$message = $_POST['message'];

// Create the email and send the message
$to = 'yourname@yourdomain.com'; // Add your email address inbetween the '' replacing yourname@yourdomain.com - This is where the form will send a message to.
$email_subject = "Website Contact Form:  $name";
$email_body = "You have received a new message from your website contact form.\n\n"."Here are the details:\n\nName: $name\n\nEmail: $email_address\n\nPhone: $phone\n\nMessage:\n$message";
$headers = "From: noreply@yourdomain.com\n"; // This is the email address the generated message will be from. We recommend using something like noreply@yourdomain.com.
$headers .= "Reply-To: $email_address";
mail($to,$email_subject,$email_body,$headers);
return true;
?>

Do you work on localhost?
If yes, do you have a mail server installed?

I did use localhost and didn’t have a mail server installed. The site is live now though, so just trying to figure out why the contact form isn’t going through? It says message has been sent, but not receiving anything.

You need NAME attributes on input and textarea fields.

<input type="text" class="form-control" placeholder="Name" id="name" name="name" required data-validation-required-message="Please enter your name.">

Try this one.

           $email="emailaddress";
            $headers = 'MIME-Version: 1.0' . "\r\n";
            $headers .= 'Content-type: text/html; charset=iso-8859-1' . "\r\n";
            $headers = 'From: Stephanie<stephanielange.com>' . "\r\n" .
                'Reply-To: stephanie@gmail.com' . "\r\n" .
                'X-Mailer: PHP/' . phpversion();

            $message = "You have received a new message from your website contact form.\n\n"."Here are the details:\n\nName: $name\n\nEmail: $email_address\n\nPhone: $phone\n\nMessage:\n$message";
            $message .= 'http://stephanielange.com';

            $subject='Hello';

            $successmail =  mail($email, $subject, $message, $headers);

            if($successmail){
                return true;
            }else{
                return false;
            }
