IOTBS style switching not showing up

Many of you are probably familiar with the ALA article Invasion of the Body Switchers. I've emailed the author(s) of IOTBS, but I thought someone here might be able to determine what I did wrong, too. This test page shows my implementation of IOTBS with the Original Integrate options. The integrated stylesheet switcher seems to work fine, but the drop-down menus that should be on the page are not showing up at all. Can anyone tell me why?

FYI, you'll notice that I have another issue mentioned on that test page relating to an unknown JavaScript/CSS conflict that I'm trying to figure out, as well. (See my thread on this issue).
