So the log says your requests to phpmyadmin were received and processed correctly.
If you go to your original code file that was showing you a blank screen,
#1: Is it still blank if you’re using localhost?
#2: Does the access log show you attempting to get the file, and if so does it show a status code 200, or something else?

I’ve gone to phpmyadmin and lo and behold the new record is inserted !!!
