I believe a higher CPM rate is based on the quality of the user experience and trust the site can develop. All other options you listed are good and practical. I think, rather than paid subscriptions, the charge mode as employed by ebay, amazon etc. is better where you only pay when you gain something from the services offered by the website. I am sure, paid subscription is not going to work with sites such as <a href="http://www.balendu.com" rel="nofollow">mine</a>.
