Yii2 - Dynamic Fields

Hi
I wanna create dynamic fields using only one table. I did for two tables and works well, but now i need to create for one table and i do not have any idea how can i do that.
My ideia is add create and edit in same controller.

I’m using this widget:

For example: I wanna duplicate all fields in table Address: Name, Street and City

I really need this.
