VERY simple review and rating script -PHP/MySQL

Hey,
Can anyone help me find a very simple review and rating script.

This is what I would like:

Give small text box and allow users to select a rating from 5-10
The page should show an initial review (Maybe I just add the first review)

Very simple script but I can’t find something with so few features

Sorry I forgot to mention, I need the script to work on multiple pages.
I am using it to rate poker sites and I want each poker site to have it’s own page with it’s own rating and comments.
