Uploading multiple images in database with same value from one field in form

Dear all,
I have a problem that I need to be helped with using one form and storing the images in database. I would like to upload multiple images with same title from “title” field.
Can somebody help me with this?

You didn’t show the structure of your database table, but it appears that you may be using $title as your key. If you want to have multiple pictures with the same title, you’ll need to create a unique key, or add a suffix to the title to make it unique, such as “Title” and “Title[2]”.
