Generally I would agree with Ryan that radio buttons should have one option set by default. But in this context, where a wrong answer could potentially result in medical complications, I can see the logic in leaving them blank and making them required.
If someone skirts through the form and neglects to answer the question, the validation will flag it and ensure it is consciously answered.
