If you are looking to assign values to specfic answers and then total it up at the end (conditional fields) then have a look at these plugins:

I’ve used this plugin before and really liked it. The paid versions allow for emailing back to the user as a quote and even PayPal integration.

Another plugin that looks promising are a combination of two:
Contact for 7 and Conditional Fields for Contact Form 7

Contact form 7 is a very popular contact form that is widely used in the WordPress community for just setting up a simple contact form page. It looks like with the use of the other plugin, gives contact form 7 the conditional fields functionality although I haven’t tried this before, it does look like the reviews may do it justice.

I also agree with @steveallenio 's answer. You can use Contact Form & and it’s Conditional Fields It’s best option if you want to generate only one form and plus this plugin is free.

I also have the alternate option if you are looking for an easy way, you can use

They are providing the really very easy interface for generating any complex form. It’s paid plugin but it’s really worth if you have to generate multiple and complex forms.
