My website homepage has a page rank 4 and I’ve .ca domain website. When I search for the keyword drug rehab my site is not showing up in google for the 1st 1000 results. could anyone tell me what is it problem and why it is not listed in google search engine ?

How old is the site. I don’t think, someone can correct me, but PR has nothing to do with ranking in the SERPS
Are you looking manually on google or using a ranking website. Are you searching on the right google site .eg .co.uk / .au
Have you set the location in tools correctly (which I doubt matters)

A good starting point would be to tell us the URL of your website 
My domain is end with .ca and pagerank is PR 4. But still not ranking in google.com. How???

I searched for “drug rehab” and Google reports there are about 35,400,000 results. There is obviously a good deal of competition and if you want your site to rank highly, you’ll have to work hard at making it one of the best. Is your site targeted at Canada, as you have a .ca domain? A search for “drug rehab Canada” shows about 3,590,000 results. Look at some of the sites that are ranking above yours, see what they’re doing well and see if that gives you ideas how to improve yours.
