Mod rewrite not working and throwing 404 error while accessing it

I am trying to extract the parameter from the url using mod rewrite. When I am accesing the page with domain.com/test/hello it is not working.

Options +FollowSymLinks
RewriteEngine On
RewriteBase /test/
RewriteRule ^/test/([^/]*) /test/index.php?name=$1 [NC,L]

Regular url , example domain.com/test/index.php?name=hello works fine.

I even asked the hosting support but they mentioned everything looks fine. Can someone see if I am missing anything here…struggling to resolve it for last 24 hrs…

The Options +FollowSymLinks should be in the server’s configuration file and MP’s ditching of the RewriteBase directive is because it’s simply used to UNDO a mod_alias redirection (which is not evident) and causes more problems for noobies than it’s worth.
