Looking for more than just a registration form

Hi there

I’m looking for a ‘plugin’ for Wordpress whereby the user can register their contact details, then continue to the next level or page, select 1-4 options via a check box per item, then check out to pay by credit card. Does anyone know of a template plugin I can follow and complete? Or, perhaps I could use ‘Mailchimp’ to compile a template which could then perhaps be embedded into Wordpress?

Any ideas and examples would be very appreciated.

Wufoo is a pretty sophisticated service for generating forms, so I’d take a look at them and see if that’s of use.

Great thanks very much. I’ve also found a Wordpress plugin <snip/> called Form Maker. This also seems easy to set up.
