I want web design work done. Do designers just send you a graphic of the layout or do they splice it to html/css for you?

You actually just get a PSD file of the layout. At 99designs the designers design the ‘look’ of your website, they do not provide the coding.
If this is a requirement you can arrange with a coding service after you have received the design. Here is one that they recommend.

That would depend on which of the two alternatives you want. One will be more expensive than the other as it involves more work.
