Hi I am looking for a form builder, similar to plugins you can find for CMSs like WordPress.

Basically I want to avoid hand coding html forms for small apps that I create from time to time.

So I want a form builder that lets me add the fields and choose from a number of templates/colour schemes, possibly even implement some jquery based validation.

Anyone has any such good tool, preferably free? Thanks
