Force domain to www and do ip canonicalization

I try to force this domain https://okmedical.ro/ to have www in front of domain every time. And I have canonicalize the IP address. I tried to put my code in top, after the 3rd line in .htaccess but the site doesn’t work. Someone please tell me how to do it.

My code is this:
