Error from apache

I have been running the web server abyss for years with version 5.2.10 and currently upgraded to apache 2.2 with php 5.2.17. When i view a php page such as:

echo “working”;
?>

it works great but when i view my site that worked fine i get this error, does anyone know what it means, i dont know if its my site or apaches side?

Notice: Undefined index: loginname in C:\Program Files\Apache Software Foundation\Apache2.2\htdocs\index.php on line 9

Notice: Undefined index: code in C:\Program Files\Apache Software Foundation\Apache2.2\htdocs\index.php on line 10

Fatal error: Call to undefined function mysql_connect() in C:\Program Files\Apache Software Foundation\Apache2.2\htdocs\config\database.php on line 14

The first 2 errors mean that $loginname and $code both are undefined and should have an isset statement to detect if they exist within the code already.

The last error is telling you the MySQL .dll file is not enabled in your php.ini file, simple open your php.ini file and find

;extension=php_mysql.dll

and simply remove the ; from the start of it then restart your server.
