Some hotels in the US/Canada mention this:
'All rooms include full cooked to order breakfast'

I was wondering what it meant exactly

I'd imagine it means exactly what it says (as opposed to teh usual breakfast buffet.) Specifically, they probably have a few omelette chefs at the buffet table.

The Hotel business is very competitive in the United States and most likely competitive. People want specific things to stay there. One of the things they want the most is free coffee and breakfast in the morning and free drinks in the evening. Since these things don't cost much, the hotel companies through them in. For breakfast, many offer a continental breakfast (danishes, juice, coffee) or a buffet. Some offer free access to the attached restaraunt and you can order anything on the menu with a voucher or your room key. Same thing with drinks. They can offer pre-mixed drinks, wine and beer for almost no money that you get with your room.

Other hotels offer free Internet access, suites with a kitchen and office, free office services like faxing, copying and such. Most of the ones catering to business travelers also offer free dry-cleaning and laundry services.

'All rooms include full cooked-to-order breakfast' would have been clearer. I remember stuffing myself on scrambled eggs, mashed potatoes and what not at a hotel when I was in the states (I was young and erm, growing).

But what is the difference between 'full cooked to order breakfast' and a breakfast buffet?

In Europe breakfast (buffet style) is often included. You can eat/drink whatever they have there.
Maybe the difference is that you can order it and they bring it to you without having to get the food yourself?

They can either have short order cooks at the buffet tables(if they ask you what you want and make it, it's "cooked to order") or a voucher for the hotel restaurant(which would presumably include table service) as Wayne suggested.
