How to add HTML checkbox in TCPDF

I need to take report of a "Quality check list" . is there any option to use checkbox PDF as shown in pic:

Provide what code you have so we can see what you are working with. It is kind of hard to give you advice if we don't know what methods you are currently using in TCPDF.

Hi Cpradio,
I plan to use HTML option to show Radio button in pdf , but it's not showing the button. We have any hack to implement same.
