I have created a form using an online emailform company and added the code to my website.

I have just tried checking it and their site is down for maintenance which obviously doesnt look good to the client I have built the website for !!

Its a very basic form, could anyone help me with a code I could use to host the form myself somehow myself please?

This is the website, www.blitz-it.co.uk the form is in the main content on the right hand side

All i need is for them to fill in name, email and telephone number, thanks.

Do you still need help with this?
If you have PHP on the server I'd look there for a simple contact form email script, I'm sure it's a commonly asked question.
