    Need A Review, Especially From Coders, of a Designers Webpage With Form

    Just cleaning up the code, and would love a little troubleshooting help. This is the page I'm specifically focussing on, please scroll down to the bottom to see the form section:
    http://www.mindspinstudio.com/lp.php

    1.
    The items marked * need to be mandatory. Right now, if you don't select any boxes, you can enter a correct captcha and submit the form, resulting in the 'Thank you! A Mindspin representative will be in touch with you shortly" - even though no one will be in touch because no contact info was entered. Instead, a pop up window should show with the message "The page at www.mindspinstudio.com says: Please enter Name" etc. (The popup should function the same as it does on the sites other form page http://www.mindspinstudio.com/contact_mindspin.php)

    2.
    And the bullet items need to be de-selectable, so that if you click on a selected item, it becomes deselected. Currently, once you click a bullet item, you cannot 'unclick' it.

    Any other feedback on any part of the website, viewed from any desktop or device, is also welcome.

    Your title and description.
    Your title is the most important meta tag with the description being second
    I thought your title had something to do with a strategic website, why list
    storytelling

    <title>LP – Mindspin Studio – Brand Storytelling in Hamilton, Ontario</title>
    <meta name="keywords" content=""/>
    <meta name="description" content=""/>
    -

    put something like this in your description

    motivate buyers with a strategic website. The power of a strong online
    presence - which begins with a strategic website - is incredible.

    hope this helps

    I'm a web developer but explaining it here would be a bit hard, the easiest and the fastest way would be to build a form using any of those thousands form builder websites and adding the coding to your site then.

    I like the look of the site. Good color matching. There's my 2 cents. What else do you want me to test out on it?

    Please add the title of each field, instead of showing when hovered. Otherwise, it's good.
