Malware script attacked my sites

Below Malware script attacked my sites . i removed it more that 15 times but it is coming again and again . Changing the ftp passwords in 2 hour once but no use . help me to remove this script and stop its routine attack .

Your computer/one used for uploading to your site, may be infected. I had this problem about a year ago.

Every time I deleted the infected page, uploaded a clean one and changed passwords. Within a week, something was back on my site.


I downloaded Malwarebytes (from download.com) and ran it in SAFE MODE 4 or 5 times before I got a clean scan. After that I changed all the passwords again.
