save form content

on http://www.samtaleterapi.com/psykoterapi.html i would love it if people could fill in the relevant checkboxes and text fields - and save what they have entered so they can change it later

is that possible?

(if i just click save as and save the page locally, it doesnt look right when i open it in a browser - but the content is there)

any best practices?

I'm not sure if I understand your question right, and that link doesnt work. From what I gather you want to save the state of the page so the next time the user comes back its the same right? If so, the best way to do this would be to use browser cookies, check this out for more info: http://www.quirksmode.org/js/cookies.html
