Password Release Authorization

Hello- I was wondering what policies/methods others are using regarding releasing web design client's passwords or other sensitive information to third parties when requested by the client. Do you as your clients to first sign an authorization form to release you from liability? Any starting point or boilerplate language available for such a form?

I've never seen anything related to this...

In order to avoid liability, I would simply tell the client that it's up to them to distribute the confidential information to whatever 3rd parties they want. It's not your job to be a go-between.
