The icons and tools are mostly done through javascript. You just use an image or button which will execute a javascript.

This javascript will then insert some text of your choosing in the textfield. To manipulate the text appearance people use UBBC. this is a code which uses [] instead of the normal HTML <> tags. Later you parse the text of the field in PHP and replace the [] with the <> (most simplified version) and display the text.

It's a big topic to just explain in here. I would suggest google
To PHP or to Perl, that is the question!

I personally prefer YUI Rich text editor.
It's so easy to add it to your existing form, anyone can do it, even a complete beginner.
Basically you create an html form any way you want, does not even have to be in php.
Then you 'tell' the YUI RTE the name of the textarea in your form and it will then convert a textarea into a rich text editor.

There are other more advanced features for advanced users. You have to be experienced in Javascript to use advanced features.

By the way, I tried FCKEditor and TinyMCE and YUI is much better (in my opinion)

It's just that I trust YUI library to be extremely efficient. Their scripts don't take any more memory that absolutely necessary for the job.
