Near the top of the page at there's a JavaScript navigation function ("Select a Country or Location") that I'd like to use on my website.

The form looks like this:

This is the JavaScript that drives it:

But when I try to include that with my other JavaScripts, the whole page goes blank...

Can someone show me how to insert the script or suggest an alternative? Since I'm using jQuery, I thought there might be a better way to do it.
