Creating pdf and uploading it to server

I have simple form. The form contains two textbox. I want to convert it into pdf file and upload it to my server. Suppose whatever user has enter in that 2 textbox and when they click submit. The content will be written in pdf file and upload it to my server.

Any idea regarding it?

To start with, get out of the habit of thinking that PHP is running on the client. It runs on the server, so it can only create a file on the server, so there is no upload involved at all.

POST the form to a php script running on the server that uses any of the PDF libraries around to generate the file.
