Help on PHP MYSQL Forms Input

    Hi all,
I just learn PHP couple days ago, due to some 'trouble' in the company
and my boss asking me to create an employee database
Help on PHP MYSQL Forms Input

With some help of dreamweaver, i finally manage to create a form for employee database
    but.....
Sadly....some of the field cannot be recorded by the mysql...

    Can anybody analyze the problem?...

Here i upload the source code and the mysql export/dump
I'm sorry i have to use the RAPIDSHARE
because i don't where else to upload....

    yyyy://rapidshare.com/files/211344399/deesuinput.zip
    yyyy://rapidshare.com/files/211346250/deesuinput.zip

(both files are the same, replace the yyyy to http)

    Thanks in advance

    What fields?
Do you get an error? If so, what error?

Put your code (not all of it, only the parts interested, for example the form code, and the code that handles the submitted form values and stores them in the db) in your post, and wrap it in the appropriate code tags (see the buttons in the advanced editor).

Which fields are saved? Which fields not?
    Do you get an error?

Post the code of the form and the script that handles the submitted form here, in your answer.

The first 'box' and the 2nd 'box' that contained the name and personal info can be inserted to the mysql database thru that form
    but...other's not
and i don't get any error message, at all
    that's why i'm totally confused

I'm not going to download a zip file from rapidshare (or anywhere else). And I'm not going to ask again. Please post your code here, or wait for someone else's help.
