Is all you need a form, that the user fills out, and then have the info sent to your email?
Or do you need an online application with database, that stores all dates, prices, reservations, and tells the user if there's still room, how much it'll cost him, etc?
If it's just a form, google for 'php form email' or something like that, and I'm sure you'll find tons of examples, tutorials and scripts.
If it's a whole application with database, then you'll have to start studying PHP/MySQL
    PHP: http://www.php.net/manual/en/
    MySQL: http://dev.mysql.com/doc/
And google for 'php mysql tutorial' or something like that

What kind of ideas are you hoping for us to provide? This would be pretty basic php code. If you want to write this, you will need to learn a bit of php.

    Make sure you really...
    -uploaded the file
    -uploaded to the correct directory
-are using the correct filename(it can be case sensitive, I recomend using lowercase only)
