Notebook paper tutorial for photoshop

Undoubtedly you have all see those site popping up all over the place that have the content over what looks like a piece of notebook paper, with the horizontal lines, torn edges, etc.

I want to create something like this but am really lost as to how to begin. Is there a tutorial out there that can show me how to do this?

Any suggestions are greatly appreciated!

You can always take a real piece of paper, torn, notes and all, scan it in. That is a very easy method, you can test many shapes in a short time
