Will you be using only one affiliate system? Such as amazon associates, or will you be using multiple such as Commission Junction, Text Link Ads Affiliate, etc.?

Do you want to control which product/ad shows up? Do you want it to be random?

I use the text widget and put the code in there. I do use the <center> </center> code to center the banner in the side bar.

I don't use any plugins for this, but just manually edit the sidebar.php file of the WP theme of my site and I insert the affiliate link or banner code there into a new line.

you can place code in sidebar.php or if you have page ad.php in your template, you can use it well.

you can just use the sidebar widget but if you want proper controls on your earnings, you can use the ninja affiliates plugin from maxblogpress( you need to pay for this)
