create Social Bookmark

Hi,
i would like to create a social bookmark leading back to my site.
I would like it to pass the url info of the page where it came from and insert it in a form.

but main issue is to have it as a code people could copy/paste from my site and enter in theres?
any ideas how to start?

what kind of url is this i mean html,javascript,php oriented code... give more detail with example so one cna help.
