Usually forms are created with CSS & html.Not on the design.You can draw up a form on a psd to make it however you want. But you have to create the actual form in CSS & html.Draw up the form in photoshop.
The html that photoshop gives you when you slice up your PSD is garbage.And so is dreamweaver.

Can I suggest this one: http://green-beast.com/gbcf-v3/ It's one of the best - i.e. most accessible and secure forms around.
For forms you're best sticking to plain old HTML and style it with CSS.
As far as I know, photoshop has no html functionality at all. It is a pure graphics program. Design a website in html and css.

I think you can design a website with html and css. A lot of the time I will use pure css/html for layout/design experimentation. I get what you mean though. But enough wordplay.
