Ticketing System to handle site requests

The site I am working on obtains changes and requests from a wide audience. I am looking for a ticket system, not helpdesk, app that will allow the submission of requests and also allow them to be edited and closed.

I was going to use an os pm application, but none of the ones I see allow the creation of custom from end forms to submit the requests.

I see a couple of ticket system suggestions, but wondering if anyone has the same situation.

Not sure what this falls into....but I'm using Kayako and am thoroughly happy at the ability to customize front end forms, language and departments.
