Simple PHP Submit/Contact Question.

Hello, I currently own x number of Pagerank blogs and am planning on buying a website where I can get customers. I'm looking to make something like this

Where the info get submitted and emailed to me.

I'm also looking for something a little more advanced, where the user visits my site, puts in their username on another forum, and their profile link. It then checks their profile for that username and then it gives them the information, which in this case would be links.

I know a bit of HTML, but nothing advanced.

Consider using a CMS, such as Joomla, Drupal or any other. They come bundled with such functionality and then some.

I was thinking about using this with wordpress, but I want this on a 1 page template.

anyone got the basis of a code for what I'm looking for?
