    The form button is simple, just

    <INPUT TYPE="file" NAME="elementName">
    inside your form.

    I have only done the actualy file transfer with php, not sure if javascript can do it or not.

    To put the file on your website, you first need to decide what server-side language you're going to use. A google search for "html upload files" brings up lots of results, like this tutorial for doing it in PHP: http://www.tizag.com/phpT/fileupload.php
    Or this one for ASP:

    Upload scripts are very easy but you do need some sort of server side processing as Raffles mentioned. I would recommend going with PHP as it has a fairly easy learning curve, there are tons of premade scripts laying around the net and the PHP community is probably the largest and most helpful of any development language.

    Google search php+upload+script and you should find many different options to get you started.
