    No Jim. I have checked that more than 10-15 times.

    Form is submitted only once.
    I have also response.write the queries but all the queries are executed only once.

    I am very very very confused what to do here
    Please help...

    I can see possibly getting duplicates in the ActivitiesAnswers table since that For each item in Request.Form loop looks a little shady to me, but other than that, nothing is jumping out at me. Unless you're clicking the submit link, then hitting enter which would give you a double submission, but that's grasping at straws.

    Perhaps add a return false; to the end of this line?

    Dave,

    Thanks for you suggestion but I have checked that many many times.
    Form is submitted only once.

    I tried response.write that query then do response.end.

    As a result of this,
    I got only one query printed on the screen but there are 2 records entered in DB.

    Is this a bug in SQL-Sevrer??

    Make up a test scenario that still shows the duplicate records.

    Take out ALL the javascript, and as many of the ASP includes as possible. Get rid of the email sending part. Your first page should only have a form. Your second page should only have the code to process the form.

    As it is, there are too many unknowns to properly diagnose the problem.

    the solution is simple -- declare a unique constraint on the column(s) that you don't want duplicated values in

    that way, the database will prevent the dupes automatically

    Thanks for your suggestion.
    Actually there is already a column available in my DB which is unique identifier.
    Still this is not working..
    It is entering different values in this column in duplicate records.

    I have also a date/time column. In duplicate records sometimes there is same date/time recorded while sometimes there is a 1 sec difference in date/time column.

    OK, wait. Is the code you provided earlier the EXACT code you're trying to run? There's no way you should be ending up with dups there with the code you provided if you took all the odd looking code out of there.

    Post the "simplified" code here please so we can look at it. I'm starting to wonder if your server isn't caching something, or you've got some triggers somewhere that you're getting hit with.

    OK, wait. Is the code you provided earlier the EXACT code you're trying to run? There's no way you should be ending up with dups there with the code you provided if you took all the odd looking code out of there.
    No. I have removed all the removed the email sending part and all the other code from 2nd page. Now on the 2nd page it is just taking form values and then enter them into DB.

    OK, I see you've taken the answer_activity.asp page down to bare bones - have you done the same with index.asp? index.asp is most likely (like 90% chance) where the actual problem is occurring, not in the activity page.

    What is probably causing you some grief is the scripting that is occurring before the html tag. That always causes problems since most of that should be inside the <head></head> section or at the very least in the body.

    Can i just say you've got some serious tag soup brewing there? There are table tags which do not need to be there (most of the tbody tags for example), and a lot of inline css attempts which should be incorporated into a stylesheet. In fact, previewing the page, this page could be done in about half the amount of html tags, which makes maintenance much easier.

    If I may make a suggestion - once this problem is solved, you might want to go back an look at how you're building this page.

    Actually I have checked many times...
    Problems is there in where query is submitted to DB not in index.asp page
