Need helping finding the right survey software

Hello,

I'm looking to run a survey on my site. So far I've taken a look at iSalient (http://www.isalient.com/) and SurveyMonkey (http://ww.surveymonkey.com) and both look quite good.

However both seem to have limitations. SurveyMonkey cannot be hosted on my site, so I cannot have www.mydomain.com/survey.php and iSalient, although it can be hosted on my server, appears to be limited in customisation.

I would like to know if there are any survey scripts out there (preferably written in PHP) than can be hosted on my own server and can be customised in look and feel.

I'd appreciate your input.

We considered a number of PHP systems for our survey but none were any good. We ended up using Survey Monkey and it is a great system - quick and easy to set up and use!

Its open source and has been used by the US Gov. It is VERY powerful. I enjoyed playing around with it.
