Online Survey/Poll

I need some help, anybody got any ideas.
I need to create a online survey/poll.

The requirements are:

1. 30 multiple choice questions a.b.c.d.e

2. Have the ability to choose other/ provide text box to caputer "other" answer

3. data into a database (preferably Access)

4. display the results to the Administrator as a. 35% b. 25% c. 2% d. 8% e. 30%

Are there any scripts, tools, or services out there for this task.

VoteMaster could easily be modified to do what you need it to do. It basically does all that already.
